Disclaimer and Waiver

In any sport or physical activity there is a risk of injury, in skateboarding the risk of injury is higher than many other activities and by signing this form you acknowledge the risks and hazards of skating the mini ramp and agree to skate within your skill limitations. Skateboarding is considered an “extreme sport” and you will be responsible for providing your own protective equipment such as Helmets and pads. 
Participation Statement:
I acknowledge Skateboarding has a danger of injury or in extreme cases death, I am aware of and accept these risks and take responsibility for my own actions and involvement. 
Skateboarding can be a dangerous sport. I am participating with knowledge of the dangers involved to myself or my children, including without limitation, the risks of disability, injury, loss, damages, or the negligence or deliberate act of another person outside of yourself and I agree to accept any and all aforementioned risks.
I understand that by participating in the act of skateboarding I will be using facilities and/or areas where hazards may exist, and I am aware of and appreciate the risks which may result from my use of those facilities, either on the ramp or other spaces.
I agree not to participate in any use of the skateboard ramp under the influence of alcohol or drugs or any substance or illness that may alter balance or effect your physical ability or mental judgement prior to the lesson taking place.
I am aware that accidents or illness can occur during skateboarding and that I may be seriously injured as a result.
I acknowledge that I am solely responsible for my personal health and safety, and the personal property I bring with me.
I agree to assume all risks and to release in advance and hold harmless and discharge the COMPANY, the OPERATOR, all affiliated organizations, individuals and entities, first-aid volunteers, officials, participating communities, organizations, and all of their respective parents, subsidiaries, officers, directors, agents, employees and members from any liability and to waive my rights with respect to any and all claims for damages personal injury or property damage which I may have, or which may hereafter accrue to me as a result of my participation in skateboarding, even though this liability may arise through no fault of my own, or from the negligence or carelessness on the part of the persons or entities being released, from dangerous or defective property or equipment owned, maintained or controlled by them or because of their possible liability without fault.
